Role Overview
We’re seeking a dedicated and empathetic Primary Care Physician to join our growing team. This role is ideal for a provider who values autonomy, flexibility, and meaningful patient relationships. You’ll provide routine and preventive care, manage chronic conditions, and coordinate with other healthcare professionals—all while working outside the traditional clinic setting.
Responsibilities
- Conduct in-person primary care visits in patients’ homes and assisted living facilities
- Perform comprehensive assessments, diagnose and treat acute and chronic conditions
- Develop and manage individualized care plans
- Coordinate with specialists, caregivers, and facility staff to ensure continuity of care
- Document visits and maintain accurate medical records using EMR
- Provide patient education and support for families and caregivers
- Participate in quality improvement initiatives and case reviews
