Job Summary
As a Maternity RN Case Manager, you will play a vital role in coordinating care for high-risk maternity members, providing evidence-based education and support to promote healthy pregnancy, delivery, and postpartum outcomes. Key Responsibilities
- Provide trimester-specific and postpartum education to promote healthy pregnancy, delivery, and postpartum outcomes
- Coordinate care for high-risk maternity members and proactively engage them in managing their own health and healthcare
- Conduct outbound calls to assess members' current health status, identify gaps or barriers in treatment plans, and educate them on disease processes
- Collaborate with Medical Directors, providers, and internal and external partners to ensure comprehensive care
- Document and track findings, and create a positive experience and relationship with members and the team
Requirements
- Current and unrestricted Missouri RN license
- 3+ years of OB or NICU clinical nursing experience
- 2+ years of experience working in Medicaid and in a hospital setting, acute care, or as a telephonic Case Manager for an insurance company
- Proficiency with Microsoft Office Suite and electronic clinical records
- Willingness to travel up to 50% of time in the assigned region
Preferred Qualifications
- Certified Case Manager (CCM) or Certified Lactation Consultant (CLC)
- Experience working with high-risk maternity populations and in managed care
