This position will be based in Ulster county, NY for the first 90 days and the schedule will be Mon-Fri, 8a-4:30p, with 1-2 Saturday shifts.
After the first 90 days, this role will be FULLY REMOTE and the schedule will be either 9:30a-6p or 11:30a-8p with rotating Saturdays 8a-4:30p (every 3-6 weeks).
This position supports a healthcare call center by triaging patients and providing clinical guidance to ensure appropriate care and efficient coordination with providers and care teams.
Responsibilities
• Triage patient calls to assess medical needs and recommend appropriate levels of care
• Coordinate with physicians and care teams to support patient care and follow-up
• Schedule appointments, process medication refills, and submit prior authorizations
• Document patient interactions accurately in the electronic health record (EHR)
• Conduct outreach to patients and pharmacies to support care coordination and medication safety
Requirements
• AAS in Nursing required; BSN preferred
• Active New York State RN license with BCLS, Infection Control, and Child Abuse certifications
• 3+ years of RN experience in ambulatory, medical/surgical, ER, or physician office settings
• Experience with telephone triage and familiarity with EHR systems (EPIC preferred)
• Strong communication, clinical judgment, and computer skills; bilingual Spanish preferred
