Primary Responsibilities:
- Perform in-home physical therapy evaluations and mobility assessments
- Create individualized treatment plans centered on safety, strength, and functional progress
- Deliver hands-on therapy, gait training, and home exercise instruction
- Recommend equipment and home modifications to enhance independence
- Collaborate with nurses, caregivers, and interdisciplinary providers
Requirements:
- Active Minnesota Physical Therapist license
- Valid driver’s license and dependable transportation
- Home health experience a plus, but not required—outpatient and inpatient PTs encouraged to apply
- Passion for empowering patients through one-on-one, goal-driven care
