• Compensation : $130,000-$190,000 per year (dependent on experience)

Key Responsibilities :
• Assess patient needs and create personalized home-based physical therapy treatment plans.
• Implement evidence-based interventions to enhance mobility and alleviate pain.
• Monitor patient progress and modify treatment plans for optimal outcomes.
• Educate patients and caregivers on safe exercise practices and mobility strategies.
• Collaborate with a multidisciplinary care team to ensure excellent patient outcomes.
• Maintain timely and accurate documentation using user-friendly digital systems.
• Promote independence and deliver compassionate, supportive care to patients.
Qualifications :
• Valid and unrestricted California PT license
• At least 1 year of Physical Therapy experience
• Current AHA CPR certification
• Valid Driver's License and reliable transportation required
• Excellent communication, time-management, and clinical reasoning skills
