**Job Summary**
We're seeking an experienced Social Worker with a Master's degree in Social Work (MSW) and an unrestricted active license to join our remote team. As a Social Worker, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a population that spans the entire lifecycle, and all member contact will be done telephonically or via electronic exchange.
**Key Responsibilities**
* Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
* Research available options and resources for members with financial, transportation, safety, and housing needs
* Empower members to make positive lifestyle and medical choices
* Work collaboratively with other clinical staff to assist members with community resources for medical, financial, and behavioral health
* Conduct telephonic or electronic exchanges with members to assess their needs and develop plans to meet those needs
* Maintain accurate and up-to-date records of member interactions and progress
**Essential Qualifications**
* Master's degree in Social Work (MSW) from an accredited institution
* Unrestricted active license as a Licensed Social Worker (LSW), Licensed Masters Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Licensed Independent Social Worker (LISW) in the state of residence
* 2+ years of experience in medical social work or case management
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Strong problem-solving and critical thinking skills
* Ability to maintain confidentiality and handle sensitive information
**Preferred Qualifications**
* Experience working with diverse populations, including children, adults, and seniors
* Knowledge of community resources and services for medical, financial, and behavioral health
* Experience with electronic medical records (EMRs) and other health information technology systems
* Certification in case management or a related field
